Where does the information go?

This article will explain how all of the information completed in the filing information section is populated on the form(s).

  • Nature of Debt: Form 101 - Voluntary Petition Line 12 / 106Sum - Summary Line 7

  • Type of Debtor: Nowhere (if selected, joint debtor tab will appear)

  • Chapter of Bankruptcy: Form 101 - Voluntary Petition Line 7 / 106Sum - Summary Line 6 

  • District: Beginning of every form

  • Case Number:  Top right of every page

  • Date of Filing: Nowhere (used for locking income within the application)

  • Filing Fee: Form 101 - Voluntary Petition Line 8

  • Exemption Election: Nowhere (used to identify exemptions within the application)

  • Version of legal data applied to case: Nowhere (used for calculations within the application)

  • Estimated creditors, liabilities, assets: Form 101 - Voluntary Petition Lines 18-20

  • Debtor is a tax exempt organization:

  • Attorney: Any form requiring attorney signature

  • Office Location: Any form requiring attorney address

  • Statistical/Administrative Info: Form 101 - Voluntary Petition Line 17

  • Does debtor have any property that needs immediate attention? Form 101 - Voluntary Petition Line 14

  • Debtor resides as a tenant of residential property: Form 101 - Voluntary Petition Line 11

  • Is debtor a small business debtor (as defined by 11 U.S.C. § 101(51D))?: Form 101 - Voluntary Petition Line 13
