Start a new case by clicking the + New Case button and creating a Personal case.

Then under Client Profile > Filing Information > Chapter of Bankruptcy, you would select 11.

Then, scrolling down to Case Information, you can select Yes under Is debtor a small business debtor (as defined by § 11 U.S.C. 101(51D))?

This will them populate the option Is debtor proceeding under Subchapter V of Chapter 11?
