How to complete Federal Form 425C

NextChapter can autofill the Form 425C for Chapter 11 cases.

Where in NextChapter: Additional Documents > Forms Library

When you are in the Forms Library folder, there is a "Create" button next to Official Form 425C - Monthly Operating Report for Small Business Under Chapter 11.

When you click on "Create," a popup will appear asking you for additional information necessary to complete the form correctly. You can use the tabs at the top of the popup to navigate between the various questions on the form.


Once you have filled out all information in each part, click "Save to Client Document Storage" and a copy will save in Additional Documents > Client Document Storage. From there, you may download the form at any time.

If you need to make an edit to the form, you can go back to Forms Library and click the create button again and re-save the document.
