Line 11: Calculate your total current monthly income. Add lines 2 through 10 for each column. Then add the total for Column A to the total for Column B.

Line 12: Calculate your current monthly income for the year.

See below to find how line 12 (12a and 12b) is calculated.

Line 12a: Copy your total current monthly income from line 11 and multiply by 12.

For line 12a we are copying the total current monthly income from line 11 and then multiplying by 12 months.

Line 12b: The result is your annual income for this part of the form.

After copying the amount from line 11 and multiplying by 12, we then use that amount for line 12b which results in the annual income.
