When you generate your Chapter 13 plan, certain sections are automatically populated with data pulled directly from your case. This article explains exactly how each part of your plan is filled in, so you know how your case information needs to be entered for it to appear in the correct section of your plan.

§ I – Valuation of Secured Claim This section is controlled by whether you have added any claims under § III.C.
If you have added claims under § III.C, the plan will reflect that this provision is Included.
If you have no claims under § III.C, the plan will reflect that this provision is Not Included.
§ I – Lien Avoidance This section is controlled by whether you have added any claims under § III.D.
If you have added claims under § III.D, the plan will reflect that this provision is Included.
If you have no claims under § III.D, the plan will reflect that this provision is Not Included.
§ III.A – Payment in Full List all mortgage claims where the treatment is to pay in full through the plan, and all other secured claims where the treatment is to pay inside the plan and neither the cramdown nor 910 treatment is selected. Do not include arrearage claims here. For each claim, enter the creditor name, creditor address, account number, arrearage and payoff balances on the petition date, monthly payment amount, and interest rate. Indicate whether the claim is secured by real property (principal residence or other real property) or personal property/vehicle. For real property claims, enter the collateral address and indicate whether escrow is included in the regular payments or whether the debtor will pay taxes or insurance directly.
§ III.B – Maintenance of Payments and Cure of Default List all secured mortgage claims where the treatment is to pay conduit and the intent is not to surrender or avoid, and all secured arrearage claims where the treatment is to pay inside the plan. Include vehicles with arrearages. For each claim, enter the creditor name, creditor address, account number, arrearage balance on the petition date, payoff balance, monthly payment amount, and interest rate. Indicate whether the claim is secured by real property (principal residence or other real property) or personal property/vehicle. For real property claims, enter the collateral address and indicate whether escrow is included in the regular payments or whether the debtor will pay taxes or insurance directly.
§ III.C – Valuation of Collateral List all secured claims where the treatment is Subject to 11 U.S.C. § 506 (Cram down) or Excluded from 11 U.S.C. § 506 (910 Vehicle). This section is organized into three subsections — real property, vehicles, and other personal property.
For real property claims, enter the creditor name, creditor address, account number, collateral address, estimated total claim amount, value of collateral, amount of any senior claims, secured claim amount, and interest rate. Indicate whether the property is the principal residence or other real property, and whether escrow is included in payments or the debtor will pay taxes or insurance directly.
For vehicle claims, enter the creditor name, creditor address, account number, VIN, collateral description, estimated total claim amount, value of collateral, amount of any senior claims, secured claim amount, and interest rate. Indicate whether the debt was incurred within or outside 910 days before the petition date.
For other personal property claims, enter the creditor name, creditor address, account number, collateral description, estimated total claim amount, value of collateral, amount of any senior claims, secured claim amount, and interest rate. Indicate whether the debt was incurred within or outside 1 year before the petition date.
If there are no claims in any subsection, that subsection will indicate "None."
§ III.D – Lien Avoidance List any claims where the intent is to avoid the lien. For each claim, enter the creditor name, creditor address, account number, collateral description, estimated lien amount, value of the collateral, amount of any claims senior to this creditor's claim, total value of claimed exemptions on the property, and the secured claim amount remaining after avoidance (if any) with interest rate. If there are no such claims, this section will indicate "None."
§ III.E – Surrender of Collateral List any claims where the intent is to surrender. For each claim, enter the creditor name, account number, and description of the collateral (address for real property, or vehicle/asset description for personal property). If there are no such claims, this section will indicate "None."
§ III – Direct Payments by Debtor List any secured claims where the treatment is to pay outside the plan. For each claim, enter the creditor name, last four digits of the account number, and description of the collateral. If there are no such claims, this section will indicate "None."
§ IV.B – Priority Tax Claims List any priority tax claims. For each claim, enter the creditor name and total balance due. If there are no such claims, this section will indicate "None."
§ IV.C – Domestic Support Obligations List any priority domestic support obligation claims. For each claim, enter the creditor name, payment address, total balance due, and regular monthly payment amount (if applicable). This section will always appear — if there are no DSO claims, the section will indicate "None" with a blank entry row.
§ IV.D – Other Priority Claims List any priority claims that are not tax claims or domestic support obligation claims. For each claim, enter the creditor name, payment address, total balance due, and regular monthly payment amount (if applicable). If there are no such claims, this section will indicate "None."
§ V.C – Separately Classified Unsecured Claims List any unsecured claims with special treatment. For each claim, enter the creditor name, payment address, account number, basis for separate classification, and monthly payment amount. If there are no such claims, this section will indicate "None."
§ VI – Executory Contracts and Unexpired Leases List any lease claims. For each entry, enter the creditor name, collateral description, account number, and indicate whether the intent is to assume or reject. If there are no such claims, this section will indicate "None."
