When you generate your Chapter 13 plan, certain sections are automatically populated with data pulled directly from your case. This article explains exactly how each part of your plan is filled in, so you know how your case information needs to be entered for it to appear in the correct section of your plan.

§ 3(a) – Priority Claims List all priority claims that are not domestic support obligation claims. The first row will always reflect the attorney's name, bar number, and the balance of attorney fees owed. For each additional claim, enter the creditor name, proof of claim number, type of priority, and amount to be paid by the trustee.
§ 3(b) – Domestic Support Obligations List all domestic support obligation claims that have been assigned to or are owed to a governmental unit and will be paid less than the full amount. For each claim, enter the creditor name, proof of claim number, and amount of the claim to be paid. If there are no such claims, this section will indicate "None."
§ 4(a) – Secured Claims Receiving No Distribution from Trustee List any secured claims where the treatment is to pay outside the plan or pay outside the plan by a co-debtor. Do not include mortgage claims where the arrears are listed in § 4(b). For each claim, enter the creditor name, proof of claim number, and description of the secured property. If there are no such claims, this section will indicate "None."
§ 4(b) – Curing Default and Maintaining Payments List any mortgage arrearage claims or vehicle arrearage claims where the intent is not to surrender or avoid. For each claim, enter the creditor name, proof of claim number, description of the secured property and address (if real property), and the arrearage amount to be paid by the trustee. For vehicles, include the mileage. If there are no such claims, this section will indicate "None."
§ 4(c) – Allowed Secured Claims — Cramdown List any secured claims where the treatment is Subject to 11 U.S.C. § 506 (Cram down). For each claim, enter the creditor name, proof of claim number, description of the secured property and address (if real property), allowed secured claim amount, present value interest rate, dollar amount of present value interest, and total amount to be paid by the trustee. For vehicles, include the mileage. If there are no such claims, this section will indicate "None."
§ 4(d) – Allowed Secured Claims Excluded from § 506 List any secured claims where the treatment is Excluded from 11 U.S.C. § 506 (910 Vehicle), and any mortgage claims where the treatment is to pay the mortgage in full through the plan. For each claim, enter the creditor name, proof of claim number, description of the secured property, allowed secured claim amount, present value interest rate, and amount to be paid by the trustee. For vehicles, include the mileage. If there are no such claims, this section will indicate "None."
§ 4(e) – Surrender List any claims where the intent is to surrender. For each claim, enter the creditor name, proof of claim number, and description of the secured property. For vehicles, include the mileage. If there are no such claims, this section will indicate "None."
§ 5(a) – Specially Classified Unsecured Claims List any unsecured claims with special treatment. For each claim, enter the creditor name, proof of claim number, basis for separate classification, treatment, and total amount to be paid through the plan. If there are no such claims, this section will indicate "None."
Part 6 – Executory Contracts and Unexpired Leases List any lease claims. Do not list lease arrears as separate claims. For each entry, enter the creditor name, proof of claim number, nature of the contract or lease, and the treatment and intent under § 365(b). If there are no such claims, this section will indicate "None."
