Each tag has a set of instructions, which describe the logic within the app, so that you can easily understand how this tag is populated. List the employer’s information for any income source where the box is checked for “Use for Payroll Deduction”. If the income source is for Debtor, list the employer in the section under “Debtor 1 (for payroll deduction)” and if the income source is for Spouse, list the employer under “Debtor 2 (for payroll deduction)”. If there is no checkbox checked, leave the table blank. This table should have no border and should match the formatting below.
